Problem Codes

What are problem codes, and what do they mean? Problem codes are used in the Report an Issue and Paid Service Request forms to identify the issue/request and what unit(s) can provide the appropriate services. Below are the codes and descriptions, grouped into similar categories, you will see when "Reporting an Issue" or submitting a "Paid Service Request." Click on a category to see codes and descriptions related to that category.

Report an Issue

Cleaning and Landscape Issues

Below are problem codes you will see when "Reporting an Issue". In the dropdown area of the form you can start typing the problem code below and it should move to the top of the list.

SPILL-CLEANSpills/overflows/water cleanup/bio cleanup (mop, hazard, throw up, blood, puddle)
STOCK-RESTROOMRestock toilet paper, hand towels, soap, and other restroom, bathroom, custodial supplies. (tissue, trash bag, roll, liner)
DIRTY-CLEANCleaning of fixtures, floors, walls, furniture, building exterior including trash that needs to be removed. (vacuum, dust, debris, mop, grime, dirt)
LANDSCAPE-ISSUEDowned limbs, broken sprinklers, exterior trash, exterior dead animals, or other issues with landscaping (plant, flower, tree, bush, grass, irrigation)
PEST-CONTROLPest/animal issue such as rodents, insects, or dead animal inside building. (ant, beetle, bug, fly, flies, mosquito, rat, mouse, scorpion, cricket)
Electrical, Mechanical, and Plumbing Issues

FIRE-ALARMFire alarm/smoke detector not functioning, beeping, falling off wall, or needs adjustment/calibration.
LEAKLeak in ceiling, wall, pipe, faucet, commode. (dripping, flood, toilet, urinal)
ELEVATOR-ISSUEElevator/escalator not functioning properly.
LIGHTSLights flickering, light sensor or timer not functioning, or light out (bulb, lamp, switch)
PLUMBING-ISSUEClogged/stopped-up toilet, urinal, drain, or sink. Toilet/urinal continually flushing/filling. Broken faucet - won't shut off or turn on. (commode, drinking fountain, water, running)
Building Comfort and Pest Control Issues

PEST-CONTROLPest/animal issue such as rodents, insects, or dead animal inside building. (ant, beetle, bug, fly, flies, mosquito, rat, mouse, scorpion, cricket)
Building Structure, Key, and Equipment Issues

DAMAGE/MISSINGDamaged or missing walls, windows, ceilings, handrails, furniture, broken, loose, cabinet, hole, railing, fixture, dispenser, hole, etc.  Damage may be a result of vandalism, graffiti, accidents, misuse, unknown, etc.

POWER-WASHRequest for power washing/pressure washing outside of regular schedule
CLEANDeep clean and general cleaning outside of routine cleaning schedule. These may include, but are not limited to, office cleaning such as dusting, polishing, and interior partition glass (Outside of annual or scheduled cleaning).
CARPET-CLEANCarpet cleaning request for service outside of normal schedule.
FLOOR-CLEANINGStrip and wax. Request for service outside of normal schedule
WINDOW-CLEANINGRequest for window, window screen, and window treatment cleaning outside of routine schedule
Additional Landscape Services

Safety and Security Request

LOCK/UNLOCKSecurity lock/unlock - for access to buildings/spaces after typical building hours.
FIRE-SAFETYDisable system for inspections. Not for use for event-related requests.
SECURTY-INSTALLCoordination with ITS on Building Access Control System (BACS) installations, keypads and card readers, security upgrades/installs, cameras, gates
KEYPAD-UPDATEExisting door keypads, updating firmware, testing batteries, programming lock
KEYS-FURNITUREAdditional or replacement keys to furniture.
DOOR-HARDWAREUpdate door hardware, replace locks
KEYS-NEW/REPLACKey issuance, new keys, replacement key, key cutting, restocking
LOCK-REPLACELock replacement for existing locks.  Includes replacing the core with no modifications to the door.
ABATEMENT-SPPTAbatement support for asbestos and lead; gasket removal
Renovation and Space Modification Request

SIGNAGESignage and wayfinding; vinyl, engraved, printed
SPECIALTY-LIGHTReplacement of display case lights, shadow box lights, specialty lighting, built-in desk lighting, tracking lighting
STUDY/PLAN-SVCSProject planning services, including space allocation and feasibility studies; Study changes and/or needs for campus facilities
SPACE-MODFacility additions, utilization changes, modification to existing space, or changes to the specified use of space
Installation and Equipment Request

LAB-EQPT-INSTALAutoclaves and fume hoods, emergency eyewash and shower stations
ELECT-REPLACEReplacement of outlet or switch. Does not include moving or installation of outlet or switch in new location.
DEPT-EQUIPMENTRepair/replace department-owned equipment, including refrigerators, ice makers, freezers, studio, and lab equipment.
APPLIANCE-DRAINPreparing appliances for disposal.  Drain freon, remove batteries
PLUMB-REPLACEPlumbing fixture replacement like for like.  Sink, faucet, shower, eye wash station, drinking fountain.  Use "RENOVATION" for different makes, models, or alterations.
POWER-INSTALLInstall or modify electrical outlet, power source, systems furniture, cameras
HANG/REMOVEHang/remove room fixtures including mirrors, shower curtains/rods, racks, tissue holders, sharps brackets, bulletin board, picture, whiteboard, cork board, glass board, etc.
KEYBOARD-TRAYSInstallation of keyboard trays on desks
BANNER-HANGHanging of large banners.  Request does not include the creation of the banner.  Please use "SIGNAGE" if the banner needs to be created.

Move Requests

Below are problem codes you will see when submitting a "Move Request". In the dropdown area of the form you can start typing the problem code below and it should move to the top of the list.

BOOK-CARTSRequest for only book movers/ book carts to be dropped off and later picked up after customer is finished using them. This does not include a move.
EXPED.-SURPLUSFrom locations on campus to Surplus Warehouse (max 4 rooms to Surplus) Note: this is a paid service requested to expedite the typical Surplus pickup request.
MAJOR-MOVEMove involving 5+ rooms and/or 1+ office suites.  If a move is associated with a renovation project, this should be requested/scheduled through the project manager.  A member of EMS will reach out to discuss the details of the move.
MINOR-MOVEA move that includes 1-4 rooms/locations being moved into 1-4 rooms/locations.  Note: Office suites are considered Major Moves.
OFFSITE-MOVEOffsite storage unit to/from locations on campus (max 4 rooms to/from offsite storage; for 5 or more rooms please choose the Major Move type)
REARRANGEMENTMoving/rearranging furniture or items within the same room
ROOM-SWAPTwo offices/rooms/locations that trade with each other.  For example:  Room A moves to Room B.  Room B moves to Room A.
SPECIALTY-MOVEHigh value; research-related; supporting curator transport; moves that require a direct transfer of items from one location to another without any stops; lab equipment relocation (note: install is not included); delivery from Document Solutions
